New Registrations

Please bring your old medical card to the surgery to register; purple registration froms are available at reception if you are unable to find your medical card. We ask all new patients to complete a questionnaire giving details of their medical history and medications. It will be several weeks before your old notes arrive at the surgery.

You can print copies of the questionnaires and complete them prior to coming to the surgery to register; they will be available at reception.

Adult Questionnaire

The completed questionnaires will help us as your medical records may take some time to reach us from your last doctor. We offer all patients a “New Registration Check” within 6 months of registration. This can be carried out by the Practice Health Care Assistant or by the doctor during other consultations. This check is very important as it enables us to obtain details of your medical history, which includes medication and allergies.

If you move

If you move your address within the practice area or change your telephone number please let us know so that we can amend your records. If you leave our practice area you will need to register with a doctor in your new practice area. If you have outstanding hospital appointments it is also advisable that you contact that hospital to advise them of your new address
